Commonwealth Psychosocial Support Program (Level 4)
Through funding from the Darling Downs and West Moreton Primary Health Network (DDWMPHN) Momentum provides free Commonwealth Psychosocial Support Program (CPSP) services which provides tailored, non-clinical support to people experiencing severe mental health challenges who are not eligible for the NDIS. Funded by the Australian Government, this program is designed to improve wellbeing, enhance social and economic participation, and build capacity for independence.
Through CPSP, Momentum offers one-on-one coaching, peer support, and group-based programs that focus on building practical life skills, managing emotional wellbeing, and increasing confidence and community connection. Each person’s journey is supported by a dedicated coach who works alongside them to develop personalised recovery goals that reflect their values, strengths, and aspirations.
Momentum’s approach is grounded in hope, respect, and empowerment. We recognise that recovery is not linear and that every person has the right to feel heard, valued, and supported in a way that meets their individual needs. Whether it’s helping someone access stable housing, reconnect with family, return to study, or simply feel less alone—CPSP plays a vital role in bridging the gap between mental health services and everyday life.
