Strengthening Pathways for At-Risk Youth and Their Carers
Building Momentum for Youth and Families is an early intervention program designed to support vulnerable young people and the people who care about them. The program combines mentoring, case management and wellbeing supports to reduce youth crime, disengagement and victimisation, while strengthening families and carers to create safer, more connected homes.
This program is proudly delivered by Momentum Mental Health in partnership with Adapt Mentorship and Mindware Psychology.
About the Program
Building Momentum for Youth and Families works with young people aged 8–17 years who may be:
- Disengaged or at risk of disengaging from school
- Involved in early anti-social or risky behaviours
- Experiencing trauma or family breakdown
- Victims of crime or at risk of contact with the youth justice system
Alongside the young person, we support their families, friends and carers, helping them build confidence, resilience and practical skills to respond to challenging situations and behaviours.
Our goal is to intervene early, strengthen protective factors and help young people and families build positive, long-term futures.
How the Program Works
Each young person and family receives tailored, flexible support, based on their individual needs.
Support for Young People
Participants receive:
- Individual case management
- One-on-one and group mentoring
- Access to creative and life-skills programs, including:
- Art therapy
- Journaling and creative writing
- Drama and storytelling
- Macramé and hands-on crafts
- Cooking and nutrition
- Mindfulness and emotional regulation workshops
Where needed, young people can also access specialist, trauma-informed counselling through Mindware Psychology.
Support for Families, Friends and Carers
Families and carers are supported through:
- Structured coaching and support sessions
- Practical skill-building workshops
- Emotional wellbeing and resilience programs
- Connection to community supports and services
Where eligible, family members may also access Momentum’s Certificate II training through the Skilling Queenslanders for Work program, at no cost.
Where We Deliver the Program
The program is delivered across Toowoomba and the surrounding region.
Support can take place through:
- Community-based programs and group sessions
- In-home and outreach support (when appropriate)
- Flexible delivery tailored to family circumstances
Our approach is designed to be safe, accessible and culturally respectful.
Program Structure
Building Momentum for Youth and Families is delivered as an ongoing 12-month program, structured into four cycles of 10 weeks.
Each year, the program can support up to 60 young people and their families.
This long-term, consistent engagement helps create stability, trust and real, lasting change.
Our Goals
We aim to:
- Reduce youth crime, anti-social behaviour and victimisation
- Re-engage young people in education, training and employment pathways
- Strengthen families, friends and carers to respond effectively to challenges
- Deliver an innovative, scalable early intervention model aligned with gold-standard practice
What We Hope to Achieve
Through participation in the program, we aim to achieve:
- Young people receiving tailored supports that address their individual risks and strengths
- Increased school and training attendance
- Stronger emotional regulation, resilience and social skills in young people
- Reduced involvement in risky or anti-social behaviour
- Increased confidence, wellbeing and capacity among families and carers
- Stronger connections between families, schools and community services
Community & Referral Pathways
Referrals into the program can be made by:
- Schools
- Queensland Police Service
- Youth Justice
- Child Safety
- Community organisations and NGOs
- Direct referrals from families through Momentum Mental Health
